Our office participates with most insurance plans, including Medicare and Triwest. Prior to making an appointment we recommend that you contact your insurance carrier to verify that the doctor you wish to see is contracted with your plan. We do not accept Medi-Cal, Medi-Medi, auto, or third party types of insurance.
Proof of insurance coverage must be presented at your appointment.
Health insurance plans may have a deductible, co-insurance or co-payment which is the subscriber's responsibility to pay. We will make every effort to verify your eligibility and coverage prior to your visit, however if upon receipt of your claim your carrier denies payment due to lack of eligibility, any charges incurred will be your responsibility to pay.
Co-payments and deductibles are due at the time of service. If you do not have insurance coverage or your coverage cannot be verified, payment is expected as services are rendered.
